Home Health Services
For those confined to their homes and who qualify for home health care, we can provide cost-effective, high-quality, individualized care and support services. Perry County Memorial Hospital Home Health Care provides these services utilizing caring, highly skilled nurses, therapists, aides, and other professionals.
The services brought to your home are individually planned, coordinated, and implemented with input from you, your caregiver, home health staff and your physician. A plan of care is developed with your physician to ensure your particular needs are met. Home health care does not require a hospital stay, but it does require a need for at least one skilled medical service listed: skilled nursing services, physical therapy, speech therapy, occupational therapy, medical social services, or home health aides.
Through the home health program, the staff actively assists and instructs patients and caregivers in the necessary care. When the home health staff is not present, patients and caregivers must provide care during this time.
PCMH Home Health Care is available in Perry, Ste. Genevieve, Bollinger and Cape Girardeau counties.
