Important Patient Information

It is the policy of Perry County Memorial Hospital to admit, and provide services to all patients without regards to race, color or national origin. The same requirements for admission are applied to all, and patients are assigned within the hospital without regard to race, color, gender, physical challenge, age or veteran status.

There is no distinction in eligibility for, or in the manner providing, any patient service provided by or through Perry County Memorial Hospital. All facilities of Perry County Memorial Hospital are available without distinction, to all patients and visitors regardless of race, color, gender, age or veteran status.

Courtesy Car

The Courtesy Car is a transportation service available to residents of Perry County who need transportation for medical purposes.  The program can be used for any services including:

  • Cardiopulmonary rehab
  • Dental & vision appointments
  • Dialysis
  • Laboratory & radiology services 
  • Outpatient surgery
  • Pharmacy
  • Physical, occupational & speech therapy
  • Physician office visits
  • Specialty clinic appointments

Hours are 7:30 a.m. to 4 p.m., Monday through Friday.

Transportation is available for medical transports only.

There is a nominal fee to use this transportation service.  Payment can be made by cash or check.

  • $4 for round trip within city limits
  • $10 for round trip within county (less than 15 miles from city limits)
  • $14 for round trip within county (15 miles or more from city limits)

Personal Emergency Response System

Personal Emergency Response System is a combination of caring people and high technology designed to improve the quality of life for those living alone, including the elderly, handicapped, and functionally impaired, allowing them to live with security and independence. Units are installed and maintained by the Perry County Memorial Hospital Auxiliary for a small monthly fee. The subscriber wears a wireless monitor as a pendant. When help is needed, the wireless monitor can be activated and a nationwide call center is automatically alerted that the subscriber needs help. Perry County Memorial Hospital is pleased to provide regular health screenings to benefit our community.

Yearly screenings include:

  • School sports physicals

PCMH offers full sports physicals for you at your convenience.  There is no limitation on dates of service nor PCMH primary care office location.  For more information, contact your doctor's office or PFCC at (573)547-7888.
